ACT@Scale – Advancing Care Coordination and Telehealth at Scale

Act@ScaleThe Region of Southern Denmark and CIMT are participating in the European project ACT@Scale. Our focus will be on dissemination activities and the upscaling of telepsychiatric services in the region.

The overall goal for ACT@Scale is to identify, transfer and scale up existing and operational Care Coordination and Telehealth good practices.

ACT@Scale has 13 European partners of which 5 regions (Northern Netherlands, Northern Ireland, Catalonia, Basque Country and South Denmark) are actively upscaling services. Each region has identified and included one or more services in the upscaling process that runs over a 3 year project period (March 2016 – February 2019). During the project period, ACT@Scale aims at reaching a total of 75,000 care recipients across regions and programmes.

As the identified services are linked to various conditions, the evaluation of the project activities will focus on upscaling processes in general and move beyond disease areas.

The project activities also reflect the areas that need to be addressed in an upscaling process. These areas include and cover:

  • Stakeholder and change management
  • Service selection
  • Sustainability and business models
  • Citizen empowerment


The project is coordinated by Philips Healthcare and serves as an example and a result of the Memorandum of Understanding made between Philips and the Region of Southern Denmark.
